Why this matters

Policies are versioned documents. A premium, excess, limit or date might be accurate for one schedule and misleading for the next renewal notice. A copied value can also lose the condition, endorsement or document page that explains it. Keeping the source nearby makes a factual workflow more honest: a reviewer sees not only a value but the material needed to check it. This is especially important before a client communication, comparison or handover, where a confident but stale note can travel farther than its source.

Record source-backed insurance facts

Capture the original referenceStore or link the schedule, renewal notice, client correspondence or other source used for the field. Record the issue date or received date so the source can be placed in context.
Describe the fact narrowlyWrite what the source supports, such as a listed excess for a named policy version. Do not expand it into a broader statement about all cover or the client’s needs.
Name the record statusUse a clear status such as extracted, confirmed, client-provided or needs review. Status is not cosmetic; it tells a recipient how much reliance is appropriate.
Retain qualificationsIf a page, endorsement, condition or missing attachment limits the meaning of the fact, link or record that qualification with the field rather than in an unrelated note.
Check before outward useBefore using the fact in a client conversation, broker handover or comparison, verify that the source is still the correct one for the work and escalate any uncertainty.

What is provenance in a policy record?

It is the record of where a fact came from, when it applied, what it covered and how it was reviewed so another person can check it in context.

Is a copied field enough to prove a policy term?

No. A copied field can lose its version, page, condition or exception. The original document remains important for checking meaning and scope.

Why label a fact as client-provided?

It makes clear that the value came from the client rather than from a policy document or broker verification, helping the next reviewer decide what to check.

Can provenance replace broker judgement?

No. It supports a better factual starting point. Professional judgement still requires the appropriate human review of the source and client context.

Is Insia Intelligence available to all brokers?
